How to Request Your Medical Records

You have the right to access and receive copies of your chiropractic records, treatment notes, and diagnostic images (X-rays). To request your records, please follow these steps:

1. Obtain the Form: Send an email to tallantchiropractic@gmail.com or text (469) 349-8148 to request a Medical Records Release Form be sent to your email address on file.

2. Submit the Request: The form will automatically be saved to your Patient Portal upon completion.​

3. Verify Identity: For your privacy, you must include a copy of a valid government-issued photo ID with the request to verify your identity.

4. Processing Time: Standard electronic records or physical copies will be processed within 1-5 business days of receiving your signed authorization.
